What is a Medical Home?
 

The American Academy of Pediatrics has recommended that every child should have a medical home.

A medical home is not a building.

A medical home is a service that coordinates the evaluation & treatment of children with medical needs.

At Maria Fareri Children's Hospital, we have created a unique type of medical home for children. This is an out-patient service, where families caring for children with special or complex medical needs may have their children seen by all of their specialists, in one day, at one location.

Each patient finishes the day with a comprehensive treatment plan, coordinated by a team of specialists. That plan is communicated to the family, and to the child's primary care physician.

The Medical Home for Children allows families and physicians to work together, to address their children's healthcare needs, in a family supportive system that emphasizes communication and continuity of care.

 

Program Description
 

The Medical Home for Children provides a comprehensive approach to the care of children and young adults with significant chronic illnesses.

We create a medical home by improving communication between families and the medical specialists.

In keeping with the American Academy of Pediatrics Medical Home Initiative, our goal is to provide accessible, family-centered, continuous, comprehensive, coordinated, compassionate, and culturally effective medical care for children with specialized health care needs.


The Medical Home for Children Provides:

  • Highly skilled nurses, physicians & support staff
  • Empowerment of patients and caregivers through education and access to specialists
  • Help with choosing and setting up appropriate referrals (medical appointments, procedures and testing) and providing appropriate background information
  • Interpretation of subspecialty recommendation and coordination of care
  • Evaluation, treatment, and referral when indicated for mental health issues that may be the primary condition or the result of a serious medical condition
  • Guidance and referral for family counseling
  • Advocacy with insurers, government programs, schools and/or workplace
  • Information on diagnosis, treatment and support services

Appointment Scheduling
 

Our services are rendered on an appointment only basis. If you must reschedule your appointment, please call (914) 594-4602 between 9:00am and 4:30pm on weekdays. If you must cancel an appointment, please call our office allowing at least 24 hours notice.  We have a very busy schedule and a missed appointment often prevents another patient from being seen.

Please allow for a minimum of 3 hours in the office. Your child will be seen by several doctors and the clinical coordinator during this time.

 

Billing Policies
 
Please make sure that all necessary referrals to the pediatric specialists are provided by your child's pediatrician prior to your appointment at the Medical Home. It is important that you inform us of any changes in your personal identifying information, such as changes in insurance, home address, or telephone number. Please bring all insurance cards with you to each visit.  If your insurance requires that you are responsible for a co-pay, we will request it upon check-in. If you are seeing multiple specialists, several co-pays may be required.
 

Release of Medical Information
 

The Medical Home for Children is legally required by the Federal Health Insurance Portability and Accountability Act (HIPAA) to assure privacy with medical information. 

You will be asked to complete an Authorization for Release of Medical Information Form in our office.

Does your child see multiple medical specialists or receive health services from various agencies?

to Sprain Brook Parkway South to the first exit - Route 100 / Hawthorne (just past the NY State police Headquarters). Make a right at the top of the exit onto Route 100 ( Bradhurst Avenue ). Continue straight to 19 Bradhurst Avenue , approximately 1/4 mile on the right.

 

ROUTE 684 SOUTH

to Saw Mill River Parkway to Sprain Brook Parkway South to the first exit - Route 100 / Hawthorne (just past the NY State police Headquarters). Make a right at the top of the exit onto Route 100 ( Bradhurst Avenue ). Continue straight to 19 Bradhurst Avenue , approximately 1/4 mile on the right.

 

SAW MILL RIVER PARKWAY SOUTH

to Sprain Brook Parkway South to the first exit - Route 100 / Hawthorne (just past the NY State police Headquarters). Make a right at the top of the exit onto Route 100 ( Bradhurst Avenue ). Continue straight to 19 Bradhurst Avenue , approximately 1/4 mile on the right.

 

SAW MILL RIVER PARKWAY NORTH

to I-287 East to Exit 4. Make a left at the end of the exit ramp. (Route 100A North) Go straight for approximately 3 miles. You will go through 4 traffic lights. At the 4th traffic light, the street name will change to Bradhurst Avenue . 19 Bradhurst Avenue will be on the left.

 

NEW YORK STATE THRUWAY ( I-87 SOUTH)

to Tappan Zee Bridge to Exit 8 (I-287 / Cross Westchester Expressway). Take I-287 to Exit 4. Make a left at the end of the exit ramp onto Route 100A North, which becomes Route 100 North or Knollwood Road . Go straight across Grasslands Road and past Blythdale Hospital for approximately 3 miles. 19 Bradhurst Avenue will be on the left.

I-287 / CROSS WESTCHESTER EXPRESSWAY (EAST OR WEST) TO EXIT 4

Traveling East - Make a left at the top of the ramp.

Traveling West - Make a right at the top of the ramp.

Onto Route 100A North, which becomes Route 100 North or Knollwood Road . Go straight for approximately 3 miles to 19 Bradhurst Avenue on the left.

 

I-95 ( NEW ENGLAND THRUWAY) NORTH OR SOUTH
